Frank Castle finds himself in a lethal predicament, handcuffed to a car rigged to explode, until a timely intervention by Shotgun provides a narrow escape. As the duo works to track down the Carbone crime family and rescue Mickey Fondozzi, the pursuit leads them to a mob summit on a remote private island. Meanwhile, the amnesiac Salvatore Carbone begins to piece together his past under the name Thorn, setting the stage for a violent collision between the vigilantes and the underworld.
